Description
Element Care, a non-profit PACE provider, seeks an Activities Specialist to design and lead engaging, therapeutic programs for older adults. In this role, you will plan group and individual activities that promote socialization, cognitive stimulation, and physical well-being for participants in our Healthcare & Medical Services department. You’ll collaborate closely with nurses, social workers, and therapists to tailor activity plans to participants’ abilities and care goals, document participation and progress, and help create a safe, respectful, and compassionate environment that supports seniors living independently at home.Plan and lead group and individual therapeutic activities for older adults in the PACE program.
Assess participants’ interests, abilities, and limitations to develop personalized activity plans.
Collaborate with interdisciplinary team members (nursing, social work, rehab) to align activities with care goals.
Document attendance, engagement, and participant responses in the medical or activity record.
Adapt programs to accommodate cognitive, physical, and sensory impairments safely.
Organize special events, outings, and holiday programs in coordination with staff and volunteers.
Ensure activity spaces, supplies, and equipment are safe, clean, and appropriately set up.
Communicate regularly with participants, caregivers, and staff to encourage engagement and feedback.
Monitor participants for changes in condition during activities and report concerns promptly.
Support a culture of respect, inclusion, and compassion consistent with Element Care values.
Activity planning and scheduling
Recreational therapy techniques
Knowledge of gerontology and aging
Behavior management with cognitively impaired adults
Documentation in healthcare or activity records
Group facilitation and engagement
Risk assessment and safety awareness
Basic vital signs/observation skills
Microsoft Office or electronic health record familiarity
Event coordination and logistics
