This is a non-dispensing clinical pharmacy position focused on comprehensive medication management and collaborative patient care. You'll work directly with providers, nurses, care coordinators, patients, and caregivers to optimize medication therapy, reduce medication-related risks, and improve health outcomes for some of the most medically complex patients in healthcare.

What You'll Do As a Clinically Integrated Pharmacist, you will:
Conduct medication reviews and identify opportunities to optimize therapy, improve adherence, and reduce adverse drug events
Collaborate face-to-face with physicians and advanced practice practitioners to develop individualized care plans
Provide evidence-based recommendations that enhance clinical outcomes and quality of life
Monitor high-risk medications and support safe transitions of care
Utilize clinical technology and data analytics to identify medication-related interventions
Educate patients, caregivers, and healthcare professionals on medication use and disease management
Contribute to quality improvement initiatives and medication safety programs
What We're Looking For
Strong clinical knowledge and passion for direct patient care
Experience in geriatrics, ambulatory care, long-term care, PACE, or comprehensive medication management preferred
Excellent communication and interdisciplinary collaboration skills

Requirements
Required Qualifications
Doctor of Pharmacy (PharmD) degree from an ACPE-accredited college or school of pharmacy
Active, unrestricted Florida pharmacist license in good standing (multi-state licensure preferred)
Current Basic Life Support (BLS) for Healthcare Providers certification
Strong clinical assessment, communication, and interdisciplinary collaboration skills
Ability to effectively manage complex medication regimens for medically vulnerable populations
Preferred Certifications & Specialized Training
Board Certified Geriatric Pharmacist (BCGP) or Board Certified Ambulatory Care Pharmacist (BCACP) preferred; certification required within 6 months of hire if not currently held
Pharmacogenomics training or certification preferred; completion required within 6 months of hire if not currently held
Minimum Clinical Experience
3+ years of direct clinical pharmacy experience with completion of a residency program, or
5+ years of direct clinical pharmacy experience without a PGY2 residency
